The Older Peoples Memory Team are excited to announce the position of a Team Clinical Lead within our teams. The successful candidate will be able to provide advice, guidance and clinical supervision to members of the Community Mental Health Team (CMHT) along with working towards individual formulations for our service users. They will also support the Senior Operational Managers with business and service delivery, whilst continuing to develop the functions and structure of the organisation to best meet the mental health needs of some of our most vulnerable clients as well as holding a small caseload.

Please note the earliest start date for this position is 01st June 2026.

Main duties of the job

You will:

  • Offer clinical leadership and maintain, monitor and facilitate the delivery of a high-quality Mental Health Service
  • Provide strong professional/managerial and clinical leadership to the multi-disciplinary team
  • Provide strong leadership in respect of developing excellence in practice
  • Ensure National Standards and local guidelines, policies and care pathways relating to care and treatment are effectively delivered
  • Promote and ensure service users and carers are fully involved in the services they receive
  • Work across N E Lincolnshire providing mental health advice and leadership based on an agreed operational policy
  • Assist in developing and evaluation of models of care to enable effective communication
  • Participate in multi-disciplinary team meetings
You will also have:
  • A Registered Nurse/Occupational Therapist/Social Worker/AHP qualification
  • Degree or equivalent
  • 1 year experience of leadership at Band 6

The post holder will be responsible for the development and implementation of the day-to-day Clinical leadership of the Team which will form part of one of the four Primary Care Networks within North East Lincolnshire. They will offer clinical leadership, ensuring the maintenance and promotion of high standards of person centred care for service users as well as assisting with the development of staff within the team.

To provide strong professional/managerial and clinical leadership to the Multi-disciplinary Team. To provide strong leadership in respect of developing excellence in practice, ensuring that robust practice and personal development initiatives are available. To ensure national standards and local guidelines, policies and care pathways relating to care and treatment are effectively delivered for the promotion and delivery of quality services. To promote and ensure service users and carers are fully involved in the services they receive.

To support good quality care, by ensuring the highest standard of environment is provided, in line with National Guidance and local service specification. To support and provide co-ordinated and integrated care, by ensuring effective working relationships exist between the services. To take the lead role in the promotion and implementation of person centred care. To ensure the care offered is at the level each one of us would want our own family to receive.

To support the staff through a change programme in line with Navigo work streams in line with the CMHT Transformation model.

MAIN DUTIES /RESPONSIBILITIES:CLINICAL:

To work across North East Lincolnshire providing mental health advice and leadership based on an agreed operational policy and evidence based practice. To assist in developing and evaluation of models of care to enable effective communication and to facilitate best practice taking into account available resources. Participate in multi-disciplinary team meetings, meetings associated with the Mental Capacity Act/Mental Health Act/Continuing Healthcare and team meetings. To monitor, evaluate and ensure maintenance of the highest achievable standards of care. Coordinate transfer or referral of service users to other teams or services where required.

Prioritise, plan and coordinate own workload and activities in line with the changing needs of the team and service. This includes planning and monitoring client medication needs, making appointments for clients, conducting assessments, managing emergencies and enquiries, responding to crisis work within individual caseload, ensuring patient records are up to date and supporting colleagues on a daily basis.

Have the ability to work independently and to be able to resolve complex professional problems. Crisis response to other staffs service users in the event of key workers absence, and delegation from manager. To participate in the implementation of an effective clinical supervision structure across the service. Assess skill mix and identify areas of development within the Team. Participate in the continued development of a comprehensive Risk Assessment and Management strategy for the service. Provide professional support to staff by devising and implementing a framework for critical incident analysis and de-brief. To provide and set up appropriate clinical training for staff across the Team.

To take managerial responsibility for the Service in the absence of the Senior Operational Manager. To be a positive team member, act as a role model for all staff grades and to question concerns in a direct and responsible manner. To ensure that all staff participate in a programme of PDR in line with NAViGO policy. To ensure that the actions and interventions of all staff are in accordance with relevant professional codes of conduct, the Mental Health Act (1983), the Mental Health Act Code of Practice and the Mental Capacity Act (2005).

To work with and support the Senior Operational Manager with sound financial approaches to finances and budgets. To ensure that appropriate systems are in place in support of all aspects of day-to-day administration and forward planning of human resources in an effective way. To assist in the development of, and to ensure the implementation and monitoring of local policies on safety. To support providing a safe environment for service users, staff and visitors through effective Health and Safety at Work awareness and provision of good standard relating to the ward environment.

Liaise with other members of the MDT and act as a link between the acute areas and Community. Participate in meetings as appropriate and act as chair for relevant meetings. Facilitate the implementation of the new model of care for community.

  • Ability to work unsupervised and deal with crisis situations.

  • Up to date knowledge of policy and procedure as it relates to service area with the ability to contribute to development of local policies.
  • Ability to demonstrate an understanding of the principles and values of clinical supervision.
  • Ability to demonstrate competence in accurate record keeping.
  • Commitment to anti discriminatory practice, with knowledge and understanding of cultural and equal opportunities issues.
  • Commitment to continuous professional development.
  • Previous experience in supervisory role.
  • Undertaken recognised Leadership/Management course.
Qualifications
  • A Professionally Registered Nurse/Occupational Therapist/Social Worker
  • IT Skills
  • 1 year experience of leadership at Band 6
  • Portfolio of relevant post qualifying education
  • Degree or equivalent
  • Non-medical prescribing qualification or willingness to work toward achievement of, advanced practitioner or responsible clinician status or willingness to work toward achievement of, including undertaking of relevant diagnostic practitioner training.
  • Post graduate qualifications in any aspect of Mental health or community care
  • Therapy qualification or experience in skill acquisition for complex mental health conditions/disorders.
  • Teaching experience/qualification
Experience
  • Degree or equivalent experience.
  • 1 year experience of leadership at Band 6
  • Multi-disciplinary working in all services.
  • Experience in case management with: sound clinical judgement broad knowledge base with commitment to evidence based practice awareness of clinical governance and quality assurance issues as they relate to mental health
  • Managing a staff team
  • Experience with working with Primary care and clear knowledge of the National CMHT Transformation programme
  • Knowledge and experience of research audit.
  • Experience of change management

£49,387 to £56,515 a year Pay dependant on experience.
