Job Purpose
The Social Work Assistant in the Community Mental Health Team (CMHT) for Older People will support the Community Mental Health Team in providing comprehensive mental health services to older adults. This role is dedicated to enhancing the well-being and recovery of citizens utilising strengths-based, person-centred approaches.
Responsibilities
- Evaluate and create individualised care plans based on a person\\\’s strengths and resources.
- Manage a caseload and offer direct support to older adults during their recovery, encouraging independence and resilience.
- Work in collaboration with multidisciplinary team members to implement strategies that facilitate mental health recovery.
- Work in partnership with people to identify their outcomes and work with social workers to implement care and treatment plans to support their identified outcomes.
Experience / Knowledge
- Experience of working with adults or people with care and support needs.
- An understanding of the issues that impact upon people who have a Mental Health condition
Desirable Requirements
- Direct work supporting adults or children with mental health conditions.
- Experience of working in statutory social care, housing, education, health or welfare advice settings.
