Transition Support Officer (Multi-Sites) Based at Hereford, with travel across National Star College sites as required. Paid at £27,780 - £28,219 per annum.
Working Hours: Full Time (52 weeks per year). Flexible working considered such as Term Time Only, Part Time and Job Share arrangements will be considered for the right candidate(s).

About the Role: As a Transition Support Officer, you will work as part of a multi-disciplinary team, providing high-quality information, advice and guidance to students, families, carers and external stakeholders throughout a learner's journey. You will act as the main transition contact for a designated caseload of students, creating personalised and aspirational transition plans that support positive post-college outcomes. Working collaboratively with local authorities, health professionals, careers services, families and internal colleagues, you'll ensure learners are equipped with the support, opportunities and resources they need to achieve their goals.
- Manage a caseload of students and provide tailored transition information, advice and guidance.
- Develop personalised transition plans that support learners from their first year through to their destination after college.
- Act as a key point of contact for families, carers, local authorities and external agencies.
- Research accommodation, education, employment, volunteering and community opportunities for students.
- Work collaboratively with Learner Journey Coordinators, Job Coaches, Careers staff and wider multidisciplinary teams.
- Prepare and maintain accurate records, reports and case notes using college systems.
- Contribute to annual reviews and support achievement of EHCP outcomes.
- Develop and maintain accessible information and guidance resources for learners.
- Participate in student reviews, transition events, focus groups and team meetings.
- Support quality assurance activities and preparation for Matrix, Ofsted and CQC inspections.
About You: We're looking for someone who is passionate about achieving positive outcomes for young people and who can confidently build relationships with a wide range of stakeholders.
Essential:- Experience of providing information, advice and guidance.
- Experience managing an individual caseload.
- Experience working with external agencies such as Careers Services, Social Services, NHS teams or local authorities.
- Excellent organisation and time management skills.
- Strong written and verbal communication skills.
- Good research and problem-solving abilities.
- Competent IT skills and experience maintaining accurate records.
- Ability to work independently and manage competing priorities.
- Level 2 English and Mathematics (or equivalent).
- Ability and willingness to travel as required.
- Level 3 Information, Advice and Guidance qualification or above.
- Experience within education, training or supported learning environments.
- Experience working with young people with learning difficulties and/or disabilities.
- Knowledge of benefits and support available to disabled people.
- Experience using databases and information management systems.
