We are currently seeking a Senior Community Physiotherapist/Occupational Therapist to join a dedicated Neuro Rehabilitation Team in South Yorkshire. The organisation is committed to providing high-quality healthcare services to individuals across South Yorkshire, focusing on a holistic approach to care and enhancing the independence and well-being of the service users through personalised rehabilitation programmes.
Role Overview: As a Band 6 Community Physiotherapist/Occupational Therapist, you will be responsible for delivering comprehensive rehabilitation services to individuals within their own homes and community settings. You will work collaboratively with a multidisciplinary team to assess, plan, and implement effective treatment plans tailored to the unique needs of each client.
Key Responsibilities:
- Conduct thorough assessments of clients’ physical and functional abilities.
- Develop and implement individualised treatment plans, including therapeutic exercises, mobility training, and adaptive equipment prescription.
- Provide education and support to clients and their families regarding self-management techniques and strategies for optimising independence.
- Collaborate closely with other healthcare professionals, including nurses, social workers, and community support workers, to ensure coordinated care delivery.
- Maintain accurate and up-to-date clinical records in accordance with organisational policies and professional standards.
Requirements:
- Degree in Physiotherapy or Occupational Therapy from a recognised institution.
- Registration with the Health and Care Professions Council (HCPC).
- Minimum of two years’ experience working as a Physiotherapist or Occupational Therapist in a community setting.
- Strong clinical assessment and treatment planning skills.
- Excellent communication and interpersonal skills, with the ability to build rapport with clients from diverse backgrounds.
- Commitment to continuous professional development and evidence-based practice.
- Valid and Clear Driving Licence.
