St Johns House Medical Centre, Worcester is looking for a committed salaried GP to join our practice team to help shape our future growth. We have strong connections to our PCN, with a great multi-generational patient population.
The Practice List size is 14,200. An innovative and supportive team comprises two GP Partners, six salaried GPs, three fully qualified ACPs, Practice Pharmacists, Practice specialist Nurses, HCAs, a Practice Manager supported by an Operations Manager, Receptionists/Call Navigators, Administrators and Secretaries. The Practice is consistently benchmarked as high performing in ICB and PCN contracts across a range of indicators, including referrals and prescribing.
Bi-weekly MDT with social and community care. Training practice.
Main duties of the job
The Role: We are offering 6-8 sessions working pattern tbc. Interests and personal development strongly encouraged. Support for being/becoming a trainer for interested candidates. Mentorship for portfolio roles, including future leadership roles in the wider community/health care.

Job responsibilities
Clinical Responsibilities: The post-holder is expected to take due note of and act in accordance with the standards of clinical governance that are agreed within the practice. Any concerns should be brought to the attention of the partners as soon as practicable. In accordance with the practice timetable, the post-holder will make him/herself available to undertake a variety of duties including: surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions, taking action on electronic prescription requests, and dealing with queries, paperwork and correspondence (including electronic correspondence), dealing with patient test results and medical reports, referral letters, paperwork and other correspondence in a timely fashion. Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers. Assess the health care needs of patients with undifferentiated and undiagnosed problems. Screen patients for disease risk factors and early signs of illness. In consultation with patients and in line with current practice disease management protocols, develop care plans for health. Provide counselling and health education. Admit or discharge patients to and from the caseload and refer to other care providers as appropriate. Record clear and contemporaneous consultation notes to agreed standards, primarily by use of the computer system and complete the clinical system clinical templates as appropriate. Collect and use data for audit purposes. Compile and issue computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible). Prescribe in accordance with the practice standards and locally agreed or national guidelines whenever this is clinically appropriate. In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
Other Responsibilities Within the Practice: Awareness of and compliance with all relevant policies/guidelines including, but not limited to, prescribing, confidentiality, data protection, health and safety, QOF, CQC. A commitment to life-long learning and audit to ensure evidence-based best practice. Contribute to evaluation/audit and clinical standard setting within the organisation. Contribute to the development of computer-based patient records. Contribute to the summarising of patient records and Read-Coding patient data. Attend training and events organised by the practice or other agencies, where appropriate e.g. CPR training. This includes clinical and non-clinical training and events. Participate in practice meetings when requested. Provide information when reasonably requested by the practice manager to ensure that all personnel and other records are accurate e.g. hepatitis status. Undertake lead responsibility for one or more clinical QOF domains as agreed with the partners/practice manager. To meet all deadlines for audits and written returns so that the practice meets quality standards and receives the designated funding (QOF, LES, DES etc.).

Qualifications Essential:

Experience Essential:
- Sound knowledge of QOF requirements, NICE guidelines etc. and commitment to deliver quality service to patients
- Confident in making decisions, managing own workload and delegating appropriately
- Good record keeping and communication skills
Desirable:
- Special interest in a specific area of primary care
