Overview
In this role you will provide senior clinical leadership for older patients with frailty across emergency services, working within the Frailty Ambulatory / Same Day Emergency Care unit and partnering with frailty practitioners, junior doctors, and therapists to deliver holistic care. You will help shape the frailty pathway, support admission avoidance and early discharge, and collaborate across departments to maintain high-quality care. The post offers a substantial clinical and leadership remit within a dynamic, expanding team. This is a permanent post at Mersey and West Lancashire Teaching Hospitals NHS Trust with a focus on improving outcomes for frail patients.
Responsibilities
- Provide senior clinical leadership in managing older people living with frailty across services
- Lead the multidisciplinary team within the Frailty Ambulatory / Same Day Emergency Care unit
- Govern medical leadership for the Acute Frailty Unit ward or participate in Virtual Ward MDT meetings
- Provide expert support to the Advanced Clinical Practitioner team within the Emergency Department
- Contribute to admission avoidance and early discharge across the frailty pathway
- Share responsibility for clinical and administrative work within the department
Key requirements
- Full GMC registration at time of appointment
- Entry on GMC Specialist Register via CCT or CESR (CCT within 6 months of interview)
- PLS, APLS or equivalent qualification
- Instructor status for one or more life support courses
