Job summaryStone & Eccleshall Primary Care Network is seeking a compassionate and motivated Wellbeing Practitioner /
Social Prescribing Link Worker to join our multidisciplinary team. This is an exciting opportunity to help people
access the support they need beyond traditional medical care, addressing the social, emotional and practical
factors that impact health and wellbeing.
Working across our PCN practices, you will empower individuals to take control of their wellbeing through
personalised support and connections to local community services, voluntary organisations and statutory
agencies.Main duties of the job Work directly with patients referred from GP practices and community partners. Conduct person-centred wellbeing conversations and assessments. Develop personalised support plans focused on what matters most to each individual. Help people access community groups, social activities and practical support services. Support individuals experiencing issues such as loneliness, financial difficulties, unemployment,
housing concerns and emotional wellbeing challenges. Build strong relationships with local voluntary, community and statutory organisations. Maintain accurate records and contribute to service improvement initiatives. EssentialQualifications GSCE grade 1 in English & Maths (or equivalent) Level 3 qualification in health, social care, community development or a related field Training in motivational coaching & interviewing or equivalent experienceDesirableCustomer/patient care experience gained from a minimum of 1 year in a general practice or healthcare environment Experience of supporting people with their mental health needs, either in a paid, unpaid or informal capacityExperience of partnership/collaborative working and of building relationships across a variety of organisations Ability to work flexibly and enthusiastically within a team or on own initiative Ability to recognise when a persons needs fall outside the scope of the role and to refer appropriately to qualified health professionals or other agencies, including for mental health concerns.
