Overview
In this role you will review and summarise clinical information from patient records and hospital correspondence onto the Practice clinical system to keep records accurate and up to date. You will work closely with clinicians to provide clear, structured histories that support safe patient care and informed clinical decisions. You will operate within NHS guidance and information governance, upholding confidentiality in a busy primary care environment. This position offers the chance to contribute to data quality, reporting, and population health within a supportive Practice team.
Responsibilities
- Code clinical information accurately onto patient records in a timely manner
- Summarise patient records into clear, clinically meaningful formats
- Provide clinicians with accessible, well-structured patient histories (on Systmone)
- Maintain data quality standards according to NHS and Practice requirements
- Contribute to improved patient outcomes through accurate record keeping and reporting
- Enter Read coded data from letters/reports into computerised medical records
Key requirements
- Experience of administrative work in a healthcare or office setting
- Experience handling confidential information
- High level of accuracy and attention to detail
- Knowledge of medical terminology
- Experience of coding and summarising
- Desirable: Experience within Primary Care; Experience of clinical coding and summarising patient records and correspondence; Experience using Systmone
