Overview
In this role you support the pharmacy team to optimise medicines and improve care for practice patients. You will conduct patient-facing clinics and clinical medication reviews, focusing on polypharmacy and chronic disease management. You will work within a multidisciplinary team, contributing to medicines safety and service development while pursuing non-medical prescribing. This is a hands-on, impact-driven position with opportunities for leadership and career progression.
Responsibilities
- Patient facing long term condition clinics and medicine optimisation
- Clinical medication reviews for patients (including care homes and domiciliary reviews)
- Medicines reconciliation on transfer of care and safety-focused prescribing
- Repeat prescribing management and policy implementation
- Risk stratification and data-driven service development
- Provide medicines information to staff and patients and address queries
- Support discharge medication processes and continuity of supply
- Medicine safety changes in response to MHRA alerts and guidelines
- Education and training on therapeutics and medicines optimisation
- Public health support and ensuring NICE/NHS guideline compliance
Key requirements
- Bachelor in Pharmacy
- Registration with the General Pharmaceutical Council
- Enroll and complete the CPPE pathway
- Minimum 2 years' experience as a pharmacist in a practice setting
- Willingness to work towards independent prescribing
- Experience in MDT/GP environment (desirable)
- Knowledge of computer systems (experience with systems like SystemOne is desirable)
- Strong communication and patient-facing skills
