Advanced Clinical Practitioner - HMP Holme House in Warminster

Spectrum have an exciting opportunity to join our team at HMP Holme House working as an Advanced Clinical Practitioner supporting our primary care team. Advanced clinical practice is delivered by experienced, registered health and care practitioners. It is a level of practice characterised by a high degree of autonomy and complex decision making. This is underpinned by a master's level award that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of core capabilities and area specific clinical competence using the multi-professional advanced clinical practice framework.

The Advanced Clinical Practitioner (ACP) will have the ability to manage clinical care in partnership with individuals, families and carers, where appropriate. This will include the analysis and synthesis of complex problems across a range of settings, enabling innovative solutions to enhance people's experience and improve outcomes. ACPs are highly experienced and educated members of the care team who are able to diagnose and treat conditions and refer onwards to appropriate specialists if needed. As such the ACP within Health and Justice will be expected to:

• Take responsibility for own learning and development by recognising and taking advantage of all opportunities to learn, in line with core professional competencies of the ACP, including full participation in appraisal and supervision, and acting as role model for self-development.
• Provide autonomous clinical assessment and treatment, demonstrating clinical expertise at an advanced level, in physical health with elements of mental health, substance misuse and learning disabilities for patients within Health and Justice.
• Contribute to the delivery of programmes of quality improvement within Health and Justice settings.
• Provide advanced and expert level of practice (including non-medical prescribing) within Health and Justice, leading to improved patient outcomes.
• Provide clinical leadership and management leading to the provision of safe, quality, effective care.

Job responsibilities: You will work alongside the Head of Healthcare to provide clinical leadership to the team.

Person Specification:

• Education and Training: Professional regulated core qualification with the NMC/HCPC/GPhC, Relevant Master level qualification in Advanced Clinical Practice, Post graduate qualification in NMP and Advanced Health Assessment, Nationally recognised mentorship/teaching qualification.
• Knowledge and Experience: Extensive experience in a management/clinical/practitioner leadership role, High level of clinical expertise in working with people with complex care needs, Extensive experience of working in primary care with a patient centred approach, Experience of developing clinical pathways and intervention plans, Experience in teaching/clinically supervising junior staff, Experience of leading/managing a multidisciplinary team, Experience of leading on a series of audits, action plans and projects simultaneously, Experience in assessing, formulating and managing risk for people with severe and enduring physical health conditions, Experience in clinically leading staff groups, Experience in developing and adapting systems and processes to meet the needs of the individual and the service.
• Skills and Abilities: Knowledge of current national agenda relating to the NHS, social care sector, and health and justice settings, Ability to involve service users in co-creation and co-production, Knowledge of evidenced based interventions when working with service users for a variety of presentations, Knowledge and good understanding of current trends and legislation impacting on services provided to those in the Health and Justice settings, Awareness of clinical governance practices, Highly developed communication skills appropriate for the client group, Evidence of experience in education of multi-professional groups, Ability to identify deficiencies in services and to actively contribute to overcoming these, Experience of mentorship in practice.
• Personal Qualities: Extensive knowledge and application in practice of leading MDTs, professional meetings, Non-judgmental attitude treating others with courtesy and respect at all times, Facilitate a culture of life-long learning including involvement in the process of clinical supervision, student final sign off and appraisal, Passion for working within Health and Justice settings, Demonstrate evidence of team working, Able to work in uncertain and challenging environments, where exposure to distressing situations is likely, Resourceful and resilient.
