Bewicke Medical Centre is seeking an enthusiastic and experienced Clinical Pharmacist Independent Prescriber to join our friendly and supportive practice team. This is an exciting opportunity for a highly motivated pharmacist who can work independently while collaborating effectively with GPs, nurses and the wider team. The successful candidate will play a key role in medicines optimisation, long-term condition management, shared care prescribing and delivery of the new Medicines LES.
Main duties of the job
- Undertake structured medication reviews and medicines optimisation clinics.
- Manage and prescribe within agreed clinical competencies as an Independent Prescriber.
- Lead on Shared Care prescribing and monitoring, including ADHD, rheumatology and other amber medicines.
- Support implementation of the NENC Medicines LES, including Shared care prescribing and monitoring.
- Injectable medicines services.
- Oral antipsychotic monitoring.
- Reconcile medicines following hospital discharge.
- Support patients with polypharmacy, frailty and complex long-term conditions.
- Respond to medication queries from patients and practice staff.
- Review pathology results related to medicines monitoring.
- Support prescribing audits, quality improvement projects and medicines safety alerts.
- Work closely with community pharmacy, secondary care and other healthcare providers to ensure seamless patient care.
- Provide education and prescribing support to clinical staff.

- Registered Pharmacist with the General Pharmaceutical Council
- Qualified Independent Prescriber.
Experience
- Experience working in General Practice, Primary Care Network or equivalent primary care setting.
- Strong clinical knowledge of medicines optimisation and long-term condition management.
- Experience undertaking medication reviews and managing high-risk medicines.
- Excellent communication and interpersonal skills.
- Ability to work autonomously and effectively manage workload.
- Strong organisational and problem-solving skills.
