b Overview /b p In this partnership role, you will lead the development and delivery of discharge and transfer of care services across GSTT and SEL. You will translate strategic priorities into actionable delivery plans and oversee performance dashboards to drive efficiency, safety and quality. You’ll build high-level relationships with senior stakeholders across organisations to enable cross‑boundary collaboration and transformative change. You will shape delivery plans and oversee coordinated workstreams to improve outcomes for patients. /p b Responsibilities /b ul li Lead strategic and operational direction for discharge/transfer of care services across GSTT SEL /li li Manage and develop a high-performing local delivery team /li li Foster strong relationships with senior clinical and non-clinical stakeholders /li li Design and lead transformational change in discharge/transfer of care processes /li li Implement and monitor programme plan with governance and dashboard /li li Utilise data to measure performance and model outcomes, coordinating with IT for data and reporting /li /ul b Key requirements /b ul li Professional registration (Nursing, HCPC, Social Care) /li li Higher professional/management degree (master’s level or in progress) /li li Evidence of relevant CPD /li li Recent senior-level experience in transfers of care and optimizing complex pathways across organisational boundaries /li li Experience managing significant organisational change /li li Experience representing services across internal and external settings /li /ul ul li Strong communication and relationship-building /li li Stakeholder engagement and influence /li li Change leadership and adaptability /li li Data synthesis and analysis from complex sources /li li Horizon scanning and best-practice identification /li li Development and use of programme dashboards and performance measurement /li /ul
