Overview
In this role you support clinical teams to deliver coordinated, patient-centered care across a PCN. You will act as a communication hub between patients and staff, organise reviews and care pathways, and help lead QOF-related improvements. You will plan health-promotion activities and ensure smooth information flow and data handling. This is a mix of clinical coordination and administrative duties that strengthens integrated services for local populations.
Responsibilities
- Coordinate rotas and information flow between services
- Maintain accurate administrative records in clinical systems (e.g., EMIS)
- Liaise with GP practices, community services and voluntary sector partners
- Support minute-taking for PCN/MDT meetings and track follow-up actions
- Monitor shared inboxes and manage escalations
- Prepare reports, audits and returns for PCN contractual obligations
- Update patient databases and care tracking systems
- Coordinate logistics for events, training and outreach
- Handle patient and staff enquiries with clear signposting
- Contribute to designing and improving internal administrative workflows
- Prepare documentation for the PCN team
- Ensure information governance, confidentiality and data protection standards
- Assist in maintaining SOPs for care coordination
- Collaborate with the PCN team on service improvement projects
Key requirements
- Level 3 qualification in administration, business, or related field or equivalent experience
- Administrative experience in healthcare, social care, or voluntary sector setting
- Strong organisational, IT and communication skills; able to manage multiple priorities
- Understanding of confidentiality, data protection, and NHS administrative systems
- Methodical, approachable, flexible, and team-oriented; committed to service quality
