Overview

In this role you will perform clinical triage of referrals (including urgent referrals), ensuring timely assessment and clear outcome communication. You will work within a multidisciplinary team to support complex referrals and contribute to integrated care coordination across services. You will collaborate with healthcare professionals, education services, voluntary sectors, and families to deliver safe, appropriate pathways for children and young people. This position offers hybrid working arrangements across Bath & North East Somerset, Swindon or Wiltshire and a mission-driven environment focused on improving health outcomes.

Responsibilities

  • Triage referrals (including urgent cases) and determine appropriate actions
  • Provide clinical advice and support for complex referral discussions
  • Coordinate care and ensure integrated pathways across services
