Main duties of the jobThe Care Co-ordinator will support the delivery of high-quality, person-centred care by coordinating services for patients with complex or long-term health needs. Working closely with GPs, nurses, the wider multidisciplinary team, community services, and external agencies, the post holder will help ensure patients receive the right care at the right time.

Key responsibilities include coordinating patient care, monitoring referrals and care pathways, supporting multidisciplinary team meetings, maintaining accurate patient records, communicating effectively with patients and carers, and signposting patients to appropriate health, social care, and voluntary sector services. The Care Co-ordinator will also support proactive patient management, contribute to service improvement, and ensure all work is carried out in line with practice policies, NHS standards, information governance, and safeguarding requirements.
