Overview
As a Clinical Pharmacist at Meir PCN, you will support medicines optimisation and safe prescribing across six practices, collaborating with GPs, nurses and the wider MDT. You’ll conduct structured medication reviews, support long‑term condition management, and contribute to quality improvement and PCN projects. The role offers opportunities to shape services, align with PCN DES and SLA requirements, and develop clinical skills within a forward‑thinking primary care network.
Responsibilities
- Conduct structured and clinical medication reviews for patients with single/multiple long‑term conditions or polypharmacy
- Identify medicines‑related risks and optimisation opportunities; monitor and manage adherence and adverse effects
- Support medicines optimisation in long‑term conditions (e.g., respiratory) and provide patient‑facing medicines advice
- Perform care home and domiciliary reviews where included in the service
- Support medicines reconciliation after hospital or other care transfers
- Provide prescribing support and respond to queries; ensure monitoring for medicines as required
- Contribute to PCN SLA/DES requirements, audits, reporting and evidence gathering
- Work with PCN MDT and practice teams to ensure consistent approaches across the network
- Engage in quality improvement, audits and service development; share learning across the PCN
- Educate GPs, nurses, ARRS and administrative staff on medicines‑related topics
Key requirements
- MPharm degree in Pharmacy; GPhC registration
- Evidence of CPD and continuing professional development
- Experience of clinical medication reviews and medicines optimisation
- Experience in supporting patients with long‑term conditions
- Experience working in a multidisciplinary team and providing medicines advice
- Experience using clinical IT systems and maintaining accurate records
- Ability to prioritise workload and meet deadlines
- Experience contributing to audits, QA or service development
- Knowledge of medicines safety and prescribing principles
- Understanding of PCN/DES environments (desirable)
