Overview
As an Advanced Clinical Practitioner or Advanced Nurse Practitioner, you will strengthen a busy GP practice in Spalding, expanding services for 22,000 patients within a Primary Care Network. You will apply advanced clinical skills to assess, diagnose and manage undifferentiated and acute conditions, including home visits, while driving service development. You will work closely with a multidisciplinary team to empower patients and improve outcomes. This role offers hands-on impact, professional growth, and a chance to shape local primary care delivery.

Responsibilities
- Assess, diagnose, treat, refer or signpost patients with undifferentiated or acute conditions
- Undertake home visits for acute conditions as assessed by the Duty Doctor
- Provide patient education to promote self-care and informed treatment choices
- Attend to first point of contact, including telephone and home visits
- Develop and support on-the-day emergency patient service
- Prioritise patients in emergencies and take comprehensive histories
- Interpret diagnostics and initiate appropriate treatment or referral
- Utilise NICE guidelines to inform case interpretation
- Signpost to appropriate services within care pathways
- Refer or discharge patients within scope of practice
- Maintain accurate documentation of patient interactions and care
- Advise on healthy lifestyles and health promotion, contributing QoF data when possible
- Facilitate learning and provide clinical advice to team
Key requirements
- Ability to manage and prioritise workload
- Autonomous working and team collaboration
- Non-medical prescribing experience and PGDs
- Experience as ECP/Advanced Practitioner in Primary or Secondary Care
- Knowledge of NICE guidelines and government targets
- Understanding of NMC/HPC Code of Professional Conduct
- Knowledge of child protection issues
- Clinical governance awareness
- Good communication and IT skills (word processing, email)
- Experience with IT-based patient documentation systems
- Strong record-keeping and triage/initial assessment skills
- Ability to interpret ECG and follow referral pathways
- Dispensing skills as per protocols and patient group directives
- Discharge planning and health education experience
- Telephone triage and advice capabilities
- Experience in health promotion and QoF data collection
- Desirable: autonomous practitioner in minor injury/illness centre or mobile service
