As a Care Co-ordinator, you'll be at the heart of a dynamic Integrated Reablement Team, supporting adults to remain at home, recover independence, and avoid unnecessary hospital admissions. This is a fast-paced, rewarding role where no two days are the same. You'll coordinate care delivery, manage caseloads, and ensure the right support is in place at the right time--helping people leave hospital sooner and stay well in their own homes. Working closely with multidisciplinary colleagues, you'll play a vital role in delivering high-quality, person-centred care across the locality.

Main duties of the job:
- Manage the reenablement caseload, ensuring work is allocated efficiently and adapting to changes in service demand.
- Create and maintain staff rosters using the electronic patient record system.
- Communicate effectively with patients, relatives, and the multidisciplinary team, using appropriate communication methods for complex or sensitive situations.
- Maintain clear, concise, and compliant documentation in line with organisational policies, including accurate patient records and consent verification.
- Provide timely information to internal and external teams, including Community Nursing, Therapy, IDS at QAH, and other agencies as required.
- Deliver clear handovers and escalate clinical risks or concerns to a registered practitioner promptly.
- Contribute to team objectives, service development, and efficient resource use, attending meetings and supporting quality improvement activities.
- Use information technology effectively to support high-quality care delivery and complete administrative tasks such as preparing notes, charts, and obtaining results.
- Participate in relevant training, apply new knowledge to practice, and reflect on the use of skills and competencies to meet evolving service demands.
- Support the delegated management and delivery of assessment, reablement, and rehabilitation services, ensuring interventions are monitored and support plans updated as goals progress.
