Overview
In this role you will provide administrative coordination and personalised care support across Taunton Deane West PCN. You will work with GPs, practice staff and multidisciplinary teams to ensure timely access to services and coordinated care. You will help patients navigate health, social care and voluntary sector provisions, contribute to personalised care planning, and support patients with complex needs to stay independent and engaged with services. This is a role at the heart of delivering patient-centred, joined-up care across primary and community services.

Responsibilities
- Book appointments for patients requiring reviews
- Process referrals, clinical documents and administrative workflows
- Maintain accurate patient records and summarise records
- Support Subject Access Requests and information governance
- Coordinate care arising from multidisciplinary team meetings and care planning
- Assist patients to access services and navigate health, social care, and voluntary sector provision
- Signpost and support self-management and patient education activities
- Support proactive identification of patients who may benefit from additional support
Key requirements
- Experience with data, databases, spreadsheets or information systems
- Experience using EMIS Web or another GP clinical system
- Excellent attention to detail and ability to work independently
- Strong written and verbal communication and confidentiality awareness
- Ability to work in a multidisciplinary team and manage competing priorities
- Knowledge of information governance and consent
- Experience in care coordination or patient navigation (desirable)
- Experience with NHS primary care or QOF monitoring (desirable)
- Experience with long-term conditions or complex needs (desirable)
- Knowledge of personalised care and support planning (desirable)
