Job summaryAn exciting opportunity has arisen for a Clinical Pharmacist to join Frome Medical Practice and Primary Care Network, supporting almost 30,000 patients across Frome and the surrounding villages. This role offers the opportunity to work within a well-established Medicines Management Team alongside experienced Clinical Pharmacists, Pharmacy Technicians, GPs, Nurses and other healthcare professionals. You will play an important role in delivering high-quality patient care while helping to shape and develop innovative services within primary care. Main duties of the jobPlaying a key role in our Medicines Management Team to ensure medicines management optimisation and safety. Undertaking structured medication reviews and chronic disease reviews with patient face to face and on the telephone, making autonomous decisions about patients medication and treatment plans. Reviewing and actioning any necessary changes to repeat prescribing and medicines management policies. Reconciling medicines following discharge from hospital, intermediate care and into care homes.
