The predicted salary is between 49000 - 54000 Β£ per year.
The post holder will work independently the majority of the time with support from the Clinical Director and GPs within the Peak Edge Primary Care Network. Engaging with GP Partners and Practice Managers across the 4 Practices covering the Peak Edge Network:
- Woodseats Medical Centre
- Steel City General Practice
- Meadowgreen Health Centre
- Sloan Medical Centre
Currently, the team works on a 12-month rotation in each surgery. The successful candidate will commence their role at Meadowgreen Health Centre (Jordanthorpe site) alongside a pharmacy technician, who they will continuously work with for any further rotations.
Main duties of the job:
- This role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver an excellent service within general practice.
- The postholder will be supported to develop their role to become a non-medical prescriber if that qualification is not already held.
Job responsibilities:
- Develop and manage a medicines management plan and deliver patient services as determined by practice/PCN policy and local and national guidance.
- Develop and manage the medicines management team, including delivery of training, to maximise cost-effective prescribing and improve the quality of patient care.
- Work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, provide face-to-face structured medication reviews, manage long-term conditions, management of medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations and reauthorisation, acute prescription requests, while addressing both the public health and social care needs of patients in the GP practice(s) that make up the PCN.
- Undertake medication review of patients with polypharmacy especially for older people, people in residential care homes and those with multiple co-morbidities.
- Provide leadership on quality improvement and clinical audit as well as managing some aspects of the Quality and Outcomes Framework (QOF).
- Provide medication review services to patients via clinics in the practice, domiciliary visits and in residential and nursing homes, and to deliver pharmaceutical care plans that maximise cost-effective prescribing and improve the quality of patient care.
- Present at patient group meetings or other appropriate events to give advice on the appropriate use of medicines.
- Produce patient information leaflets and posters and run medicine awareness projects throughout the year.
- Assist partners with the appropriate monitoring and management of their prescribing budgets.
- Prepare evidence-based resources and information to support the medicine management team and all other relevant health professionals in the implementation of rational cost-effective prescribing.
- Help plan, develop and support the introduction of new working processes within the practice to optimise the quality of prescribing.
- See (where appropriate) patients with single or multiple medical problems where medicine optimisation is required (e.g., Respiratory, Cardiovascular and Diabetes).
- Review the ongoing need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicines taking ensuring they get the best use of their medicines (i.e., medicines optimisation).
- Make appropriate recommendations to Senior Pharmacists or GPs for medicine improvement.
- Undertake clinical medication reviews with patients and produce recommendations for senior clinical pharmacist, nurses and/or GP on prescribing and monitoring.
- Work with care home staff to improve safety of medicines ordering and administration.
Person Specification:
Experience:
- Essential: An understanding of general practice and how it works, excellent interpersonal skills, excellent written and verbal communication skills, good IT skills including the ability to analyse and interpret data, attention to detail, ability to follow procedures and work to deadlines, self-motivated, ability to work flexibly and effectively, able to travel across the PCN, able to set priorities and meet deadlines.
- Desirable: Experience of working in Primary Care / GP Practice, knowledge of GP clinical systems S1 within Primary Care, experience of prescription processes and prescribing systems, experience in implementing clinical audits.
Qualifications:
- Essential: Masters degree in Pharmacy (MPharm), Registered Pharmacist with General Pharmaceutical Council, preferably 2-3 years post registration experience.
- Desirable: Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience, primary care pharmacy education pathway qualification including independent prescribing or working towards qualification.
