Urgent Community Response (UCR) Services are an integral part of the Urgent and Emergency Care pathway within Northamptonshire, focusing on avoiding hospital admissions and facilitating early discharge from hospital. This role offers an exciting opportunity to work with dedicated clinical teams and external health and social care partners to deliver this pathway and contribute to the development of UCR Services.

The services provide urgent, time-limited, multidisciplinary community-based interventions, including home visiting and remote monitoring. Timely and effective UCR services are high on the national agenda, and a clear understanding of these services and their contribution to national and local objectives is essential. We aim to create effective and high-performing teams, seeking someone proactive with proven leadership skills to provide direction and oversight.

Main duties of the job:

  • Be responsible for operational performance, including contractual, financial, quality, safety, and people, ensuring all contractual and Trust obligations are met for UCR services.
  • Support the Directorate Leadership Team to devise service plans for future service delivery and advise on the operational impact of service developments.
  • Take overall management responsibility for service delivery and operational matters for UCR Services.
  • Lead the development and reorganisation of teams when required.
  • Review and develop service models, including clinical structures, workforce planning, recruitment, training, and operationalising the service.
  • Assist with continual modernisation, redesign, and implementation of creative solutions that support National and Local initiatives.
  • Develop robust interagency relationships with key agencies supporting integrated care pathways.
  • Ensure Service Users and Carers are included in all work done by the Recovering Independence Services.
  • Work flexibly and be part of the 'on-call' and 'out-of-hours' arrangements for the Trust.
  • Provide operational leadership, holding responsibility for operational delivery of service outcomes.
  • Lead and manage the talent pool across services.
