Make a Real Impact at the Heart of Adult Community Services. We are looking for an experienced, motivated, and people-focused Senior Community Health Navigator to join our BSW (Bath & North East Somerset, Swindon & Wiltshire) Adult Community Services team. This is an exciting opportunity to take on a leadership role within a busy and dynamic service, supporting the day-to-day operational management of our Community Health Navigator and Administration Team. You will play a key role in ensuring the delivery of a high-quality, responsive service that supports patients, colleagues, and partners across Adult Community Services.
