As a Community Health Navigator , you will be the first point of contact for patients, healthcare professionals, visitors, and partner organisations, providing a professional, compassionate , and efficient service. Working in a fast-paced environment, you\'ll play a vital role in helping people access the right support at the right time while ensuring effective communication across community and hospital services .
