Overview

In this role you will practise as an autonomous Advanced Clinical Practitioner within the Rotherham UECC, spanning urgent primary care and GP out of hours. You will work in a well-supported MDT, handling undifferentiated patients and making safe referrals or discharges using evidence-based clinical judgment. The post involves extended hours including evenings and weekends, with a mix of front-door streaming, telephone triage, face-to-face consultations, and home visits. You will contribute to service development and supervise junior colleagues, shaping efficient, high-quality care across multiple pathways.

Responsibilities

  • Clinical assessment, intervention and treatment planning for undifferentiated patients (including children)
  • Autonomous referrals or safe discharge decisions
  • Work across services: front-door streaming, face-to-face and telephone triage, booked clinics, home visits
  • Support development of trainee ACPs and junior colleagues
  • Contribute to safe, effective service delivery across the wider MDT

Key requirements

  • MSc in Advanced Clinical Practice (or equivalent)
  • Non-Medical Prescribing (V300) or willingness to work towards
  • BLS certification
  • Portfolio demonstrating ACP competency
  • Ability to participate in extended-hours rota (evenings/weekends)
  • Current registration with NMC or HCPC
  • Experience with acutely ill patients and autonomous clinical practice
  • Experience in MDT working and in primary/urgent/emergency settings
  • Excellent communication skills
  • Strong organisational and prioritisation abilities
  • Ability to work effectively within a multidisciplinary team
  • Advanced clinical assessment and management planning for undifferentiated presentations
  • Clinical reasoning and decision-making under pressure
  • Experience with clinical systems (e.g., SystmOne, ED systems)
