The Hospital Business Support Coordinator supports the effective coordination and delivery of the Hospital Hub, helping ensure people are discharged from hospital safely, promptly and through the most appropriate pathway. The role acts as a key point of contact between health, social care, reablement, community services and partner agencies, promoting joined-up working to reduce discharge delays and support patient flow.

The postholder is responsible for coordinating referrals, managing the Hospital Hub inbox, screening and reviewing referral information, arranging assessments, visits and interventions, and ensuring cases are progressed efficiently. They maintain accurate records, trackers and performance data, produce reports, and help identify delays, trends and service improvement opportunities.

This role requires strong communication, organisational and analytical skills, confidence working with vulnerable people and professionals across multiple agencies, and the ability to work under pressure in a complex hospital discharge environment. Experience in adult social care and hospital discharge processes is essential.

Main duties of the job:

  • Provide leadership and operational coordination of the Hospital Hub referral pathway, ensuring referrals are managed effectively and individuals are supported to achieve safe, timely, and person-centred discharges from hospital.
  • Coordinate assessments, visits, and interventions delivered through the Hospital Hub, ensuring that resources are utilised efficiently and that service users receive timely support to meet discharge objectives.
  • Review and screen referrals, ensuring that all relevant information is available and of sufficient quality to support safe discharge planning, informed decision-making, and continuity of care.
  • Work collaboratively with health, social care, reablement, housing, voluntary sector, and community partners to support the wellbeing of service users and ensure a coordinated approach to discharge planning and post-discharge support.
  • Maintain accurate and up-to-date records, databases, trackers, and performance information relating to Hospital Hub activity, ensuring data quality, compliance, and timely reporting.
  • Produce and submit activity, performance, and management information reports as required, supporting service monitoring, operational oversight, performance management, and strategic planning.

Working closely with health and social care professionals, you will manage referral processes, coordinate assessments and service activity, monitor performance data, and act as a central point of contact for partner organisations. You will play a vital role in supporting patient flow, reducing delays, and contributing to the delivery of high-quality integrated discharge services.

The successful candidate will have excellent organisational and communication skills, strong attention to detail, and experience of managing data and administrative processes within a fast-paced environment.
