Job summaryWe have an exciting new opportunity to join our team as a Practice Pharmacist.
The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, provide structured medication reviews, manage long term conditions, management of medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations and reauthorisation, acute prescription requests, while addressing both the public health and social care needs of patients in the GP practices that make up the PCN.The post holder will act within their professional boundaries, supporting and working alongside our medicines management team. In this role, you will be supported by our Practice Pharmacist who will manage and mentor them.
As part of a multi-disciplinary team, the post holder will take responsibility for areas of chronic disease management and long term conditions management within the practice, undertaking clinical medicines reviews and address public health and social needs of our patients.
