Overview

In this role you will join a multidisciplinary PCN to provide patient-centered GP care across clinics, homes, and care home sites. You’ll work with a diverse team to support patients, carers, and clinicians, making autonomous clinical decisions and developing care plans within current protocols. The position offers exposure to a fresh role in primary care delivery and opportunities to contribute to audits, education, and service improvement. You will help shape care delivery in a collaborative, guidelines-driven environment.

Responsibilities

  • Conduct surgery and telephone consultations, home visits and care home visits
  • Assess and manage undifferentiated health problems and develop care plans
  • Prescribe within formulary and manage repeat prescriptions
  • Record clear consultation notes and collect data for audit
  • Coordinate referrals and collaborate with multidisciplinary team
  • Participate in on-call sessions and home visit duties
  • Ensure compliance with data protection, confidentiality, and information governance
  • Contribute to training, audits, and service development
  • Maintain professional development and participate in appraisal/revalidation
  • Admit/discharge patients and monitor safety and quality of care

Key requirements

  • Qualified GP with GMC registration
  • Experience in primary care and CPD
  • Experience with QOF, clinical audit, and risk management
  • Excellent communication and IT skills (EMIS, SNOMED, Office/Outlook)
  • Ability to work autonomously and as part of a team
  • Full UK driving license for home visits
  • Ability to follow clinical policy and manage workload
  • On-call and home visiting availability
  • Strong communication (oral and written)
  • Teamwork and collaboration
  • Problem solving and analytical skills
  • EMIS user skills
  • SNOMED coding
  • Office and Outlook proficiency
