Overview
In this role you will join a multidisciplinary PCN to provide patient-centered GP care across clinics, homes, and care home sites. You’ll work with a diverse team to support patients, carers, and clinicians, making autonomous clinical decisions and developing care plans within current protocols. The position offers exposure to a fresh role in primary care delivery and opportunities to contribute to audits, education, and service improvement. You will help shape care delivery in a collaborative, guidelines-driven environment.
Responsibilities
- Conduct surgery and telephone consultations, home visits and care home visits
- Assess and manage undifferentiated health problems and develop care plans
- Prescribe within formulary and manage repeat prescriptions
- Record clear consultation notes and collect data for audit
- Coordinate referrals and collaborate with multidisciplinary team
- Participate in on-call sessions and home visit duties
- Ensure compliance with data protection, confidentiality, and information governance
- Contribute to training, audits, and service development
- Maintain professional development and participate in appraisal/revalidation
- Admit/discharge patients and monitor safety and quality of care
Key requirements
- Qualified GP with GMC registration
- Experience in primary care and CPD
- Experience with QOF, clinical audit, and risk management
- Excellent communication and IT skills (EMIS, SNOMED, Office/Outlook)
- Ability to work autonomously and as part of a team
- Full UK driving license for home visits
- Ability to follow clinical policy and manage workload
- On-call and home visiting availability
- Strong communication (oral and written)
- Teamwork and collaboration
- Problem solving and analytical skills
- EMIS user skills
- SNOMED coding
- Office and Outlook proficiency
