Overview
In this role you support NHS urgent and non-urgent care pathways across North West England. You will be the first point of contact for clinicians, patients and families, directing them to appropriate services and recording data across clinical systems. Working under the Senior Care Co-ordinator, you help keep 24/7 care coordination flow and reduce waiting times. This role combines reception, booking, and multidisciplinary liaison to improve patient experience. Responsibilities
- Deliver a high-quality care coordination service to patients and healthcare professionals
- Accurately record data across clinical systems for each contact
- Complete demographics checks (e.g., NHS Spine) to ensure up-to-date records
- Book appointments across multiple services within agreed timescales
- Provide professional reception, registration and onward referral
- Coordinate follow-up care with Senior Care Co-ordinator or clinical team
- Liaise with multi-disciplinary team to ensure smooth service operation
- Coordinate with NHS 111 for out-of-area or incomplete information
- Check equipment and medications at shift start/end and report discrepancies
- Facilitate electronic data transfer with information governance compliance
- Understand clerical backup systems from computer to paper-based processes
- Comply with Complaints Policy and document incidents as required
- Undertake administration tasks and attend operational meetings
- Complete mandatory training within deadlines
- Adhere to GTD policies and procedures in decision making
- May undertake travel to other sites as needed

- Qualifications: good standard of education; Desirable NVQ Level 2 in Health Care
