b Overview /b p In this role you support the pharmacy team to optimise medicines and improve care for practice patients. You will conduct patient-facing clinics and clinical medication reviews, focusing on polypharmacy and chronic disease management. You will work within a multidisciplinary team, contributing to medicines safety and service development while pursuing non-medical prescribing. This is a hands-on, impact-driven position with opportunities for leadership and career progression. /p b Responsibilities /b ul li Patient facing long term condition clinics and medicine optimisation /li li Clinical medication reviews for patients (including care homes and domiciliary reviews) /li li Medicines reconciliation on transfer of care and safety-focused prescribing /li li Repeat prescribing management and policy implementation /li li Risk stratification and data-driven service development /li li Provide medicines information to staff and patients and address queries /li li Support discharge medication processes and continuity of supply /li li Medicine safety changes in response to MHRA alerts and guidelines /li li Education and training on therapeutics and medicines optimisation /li li Public health support and ensuring NICE/NHS guideline compliance /li /ul b Key requirements /b ul li Bachelor in Pharmacy /li li Registration with the General Pharmaceutical Council /li li Enroll and complete the CPPE pathway /li li Minimum 2 years experience as a pharmacist in a practice setting /li li Willingness to work towards independent prescribing /li li Experience in MDT/GP environment (desirable) /li li Knowledge of computer systems (experience with systems like SystemOne is desirable) /li /ul ul li Strong communication and patient-facing skills /li li Team collaboration within a multidisciplinary setting /li li Problem-solving and critical thinking /li li Medicines optimisation /li li Clinical medication reviews /li li Audit and data interpretation /li /ul
