Overview
In this role you will manage a complex caseload of adults with severe mental illness within an integrated Community Mental Health Team. You will act as Lead Professional, coordinating person-centred care plans, risk management, and multidisciplinary collaboration to support recovery and independence. You will conduct comprehensive assessments, deliver recovery-focused interventions, and contribute to safeguarding and service development. You will mentor and supervise colleagues and work closely with service users, carers, and external partners. This role combines clinical leadership with hands-on care in a mission-driven NHS setting.

Responsibilities
- Manage and coordinate a caseload of adults with severe and enduring mental health conditions
- Act as Lead Professional, developing and reviewing person-centred care plans and risk management plans
- Undertake comprehensive mental health, risk and needs assessments
- Deliver recovery-focused therapeutic interventions promoting independence, choice and wellbeing
- Collaborate with service users, carers, community services and multidisciplinary teams for effective care coordination
- Maintain accurate clinical records and contribute to service performance and outcome monitoring
- Prepare professional reports and participate in multidisciplinary meetings, reviews and case conferences
- Provide mentorship, support and supervision to students, trainees and junior staff
- Participate in safeguarding, clinical audit, quality improvement and service development activities
- Ensure care aligns with legislation, professional standards, Trust policies and best practice
Key requirements
- Registered Mental Health Nurse (RMN) with current NMC registration
- Evidence of ongoing professional development and commitment to maintaining competencies
- Knowledge of safeguarding, Mental Health Act, Mental Capacity Act and Care Programme Approach processes
- Strong assessment, risk management and care coordination skills
- Excellent communication, organisational and multidisciplinary working abilities
- Competence in maintaining accurate clinical records and producing professional reports
- Desirable: Mentorship, practice assessor or supervision qualification; additional training in psychosocial interventions, risk assessment, safeguarding or therapeutic approaches; CMHT experience
