In this role you join a small multidisciplinary Mental Health Outreach team based in Portsmouth to support people with co-occurring mental health and substance misuse needs. You will work in the community, co-located with substance misuse services, and act as an access point to appropriate services to improve health, reduce harm and support recovery.
You will collaborate with carers, other professionals and agencies to plan and deliver evidence-based care, with a strong focus on recovery. A meaningful development path, including leadership training and advanced courses, awaits within the Trust.
- Conduct community-based assessments and joint planning with service users and carers
- Provide short-term, evidence-based interventions and tailored care plans
- Deliver risk assessment and safety planning within multidisciplinary settings
- Collaborate across statutory and voluntary agencies to build a robust network of support
- Work autonomously and as part of a team to ensure timely access to services and prevent exclusion

Key requirements:
- Registered Mental Health Nurse, Social Worker or Occupational Therapist degree-level qualification or postgraduate equivalent
