Overview
In this role you will join DDPCN's Home Visiting Service to deliver high-quality clinical assessments and care in patients’ homes.
You’ll work within a multidisciplinary primary care team, supported by GPs and colleagues, to improve access and reduce hospital admissions.
You will manage treatment plans, investigations, and escalation of deteriorating patients, while promoting self-management and prevention.
This is a patient-centered, autonomous yet collaborative position in a beautiful, reachable Derbyshire setting.

Responsibilities
- Conduct home-based clinical assessments and document histories
- Develop and implement management plans within scope of practice
- Request and interpret investigations and escalate when needed
- Prescribe via delegated pathways per local regulations
- Collaborate with GPs, nurses, paramedics, pharmacists, and community teams
- Maintain accurate clinical records and support governance, audits, and quality improvement
- Promote patient education, self-management, and preventative care
- Key requirements
- MSc or Postgraduate Diploma in Physician Associate Studies or equivalent
- Successful completion of Physician Associate National Examination
- GMC registration as a Physician Associate
- Experience in primary or urgent care settings and in MDT work
- Excellent clinical assessment and diagnostic skills
- Strong communication and interpersonal skills
- Independent working with appropriate support seeking
- IT proficiency and experience with electronic patient records
- Understanding of safeguarding for adults and children
- Good organisational and time-management skills
- Ability to travel across the service area
