Job summaryThe
Social Prescriber will work as part of the multidisciplinary PCN team to
support patients with non-medical needs that impact their health and wellbeing.
The role involves person-centred conversations, identifying what matters to
people, and connecting them to community-based support, voluntary services, and
local resources across the PCN footprint.The
post holder will work across five GP practices, ensuring equitable access to
social prescribing services for patients and supporting population health
