Overview

In this role you support medicines optimisation and safe prescribing across up to two PCN practices within a governance-enabled team. You will perform structured medication reviews, contribute to long-term condition management, and enhance medicines safety and patient outcomes. The role is funded via ARRS and sits within Roby Medical Centre, offering flexible hybrid working and strong professional development. You will collaborate with GPs, nurses, and the broader team to drive high-quality pharmaceutical care. This is a permanent post with a clear path for clinical oversight and supervision.

Responsibilities

  • Undertake Structured Medication Reviews (SMRs) for patients with polypharmacy, frailty, and high-risk medicines
  • Provide expert medicines optimisation advice to GPs and the multidisciplinary team
  • Prescribe and deprescribe within scope of practice per NICE guidance and local formularies
  • Support long-term condition management across cardiovascular disease, diabetes, respiratory disease, and frailty
  • Monitor and manage high-risk medicines (anticoagulants, DMARDs, controlled drugs)
  • Contribute to antimicrobial stewardship and prescribing quality improvement
  • Participate in audits, significant event learning, and PCN medicines safety activity
  • Collaborate across practices to ensure consistent, high-quality pharmaceutical care

Key requirements

  • GPhC registered pharmacist
  • Independent Prescriber qualification (or completion by start date)
  • CPPE Primary Care Pharmacy Education Pathway (PCPEP) completion (or at least 80% completed) with evidence
  • Experience in primary care or general practice
  • Strong understanding of NICE guidance, medicines optimisation, and prescribing governance
  • Ability to work flexibly across more than one PCN practice
  • Confident using GP clinical systems (EMIS)
