The Patient Safety Incident Response Framework (PSIRF) has now been implemented within Sandwell and West Birmingham Hospitals NHS Trust (SWBH) for two years and our Patient Safety Investigators play a key role in the continued embedding and development of our processes. We are looking to recruit an enthusiastic individual with a passion for improving the safety and experience of our patients, people and population to join our team and help us continue to learn and improve.

As a Patient Safety Investigator within the Corporate Patient Safety Team, you will work closely with the Clinical Divisions to promote a culture of openness and learning from good practice and areas that may need some improvement. You will lead on a range of incident investigations looking at systems and processes using the SEIPS tools and support clinical staff to understand and use a range of learning tools including swarm huddles, After Action Review and Multidisciplinary Team reviews.

Supported by the Quality Improvement team you will be part of a team to recommend and implement improvements across the Trust, working across a wide range of teams. You will also engage with patients and their families to understand their perspective as part of your investigations, ensuring their experience is at the centre of all improvements made. We want someone to embody our Trust values of Ambition, Respect and Compassion whilst delivering our Strategy: Our Patients, Our People, Our Population.

Main duties of the job include:

  • Working closely with clinical divisions, across a wide range of specialties and clinical grades.
  • Adapting your communication style to engage in discussion with team members, showing empathy and compassion during difficult conversations.
  • Demonstrating excellent verbal communication and effective written/digital skills to provide appropriate assurance and updates regarding your areas of responsibility.
  • Having a natural curiosity to delve into systems and processes to understand how things may need to be improved.
  • Analysing data in various formats to horizon scan and anticipate any issues before they are encountered.
  • Creating and delivering learning materials in various formats to local areas, engaging in research for patient benefit and supporting the development of policies.

About us: Sandwell and West Birmingham NHS Trust (SWB) serves Birmingham and the Black Country - one of the country's most culturally diverse areas. It's a friendly and welcoming place - a place where you can belong, and a place where you can grow. We care for our patients, we care about our population, and we care about our people.

Our values - Ambition, Respect and Compassion - are at the heart of who we are. They guide us every step of the way; how we work with each other, and how we look after our patients and their families; how we respect and value the rich diversity of our team and our community.

Our Trust has always aspired to be more than a hospital, more than a healthcare provider. Our purpose is to "Improve the Life Chances and Health Outcomes of our Population." It is what inspires, drives, and unites us every day. It's what makes us unique.

We want working at SWB to feel like more than just a job. We want our Trust to be a place where you can feel you belong. A place where you feel happy, safe, and rewarded. A place where you can develop your career in whatever way you choose.

When we say we're 'with you all the way' we want you to know that we mean it.
