Position available up to full time, part time position considered. Provides safe, effective and evidence-based pharmaceutical care, including independent prescribing, to optimise patient outcomes. Works autonomously, applying advanced clinical knowledge and judgement to assess, diagnose and manage patients within scope of practice. Role Accountabilities
- Contribute to the delivery of safe, effective and evidence-based clinical assessment, diagnosis and prescribing within scope of practice.
- Apply advanced clinical knowledge and judgement to determine appropriate treatment plans, including medicines optimisation and deprescribing where required.
- Manage a defined caseload, prioritising patients based on clinical need and service requirements.
- Contribute to safe and effective use of medicines, ensuring prescribing aligns with clinical guidelines and best practice.
