Main duties of the jobYou will join a team of Clinical Practitioners from nursing and paramedic backgrounds dealing with many common health concerns, including minor illnesses and long-term conditions. We are a SystmOne user and use SystmConnect to provide GP total triage to patients for all appointments. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate. Provide health promotion and education to patients around self-care and appropriate use of services. Work collaboratively with GPs, practice nurses, pharmacists, and wider primary care teams. Maintain accurate and timely clinical documentation in line with practice and legal requirements. Participate in clinical audits, reviews, and quality improvement activities. Be aware of health inequality and government agendas such as Core20PLUS5 with the aim of reducing health disparities between groups of people and improving access to healthcare. Adhere to safeguarding policies and procedures to ensure patient safety. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Support patients to adopt health promotion strategies that promote patients to live healthily, and apply principles of self-care.
