Advanced Clinical Practitioner in Nottingham

An exciting opportunity has arisen for an experienced Advanced Clinical Practitioner to join our Virtual Ward and Reablement Team within Nottingham CityCare Partnership. The Advanced Clinical Practitioner plays a pivotal role in delivering rapid, high-quality clinical assessment and intervention for adults experiencing an acute health or functional crisis in the community. The ACP provides expert autonomous practice to prevent avoidable hospital admissions, support safe and timely discharge, and enable effective reablement, ensuring people can remain safely at home wherever possible.

The postholder leads clinical decision-making within a multidisciplinary team, provides advanced assessment and treatment planning, prescribes medication where appropriate, and contributes to service development, staff training, and integrated working across health and social care partners. The ACP carries clinical responsibility for a complex caseload, applying advanced investigative and analytical skills to lead safe, evidence-based practice, support timely discharge planning across community settings, and provide specialist advice to colleagues across the multidisciplinary team.

The ACP is accountable for independent clinical assessment, diagnosis and treatment of patients with undiagnosed and undifferentiated conditions, practicing at an advanced level demonstrating in-depth knowledge and competence in all aspects of UCR/Virtual Wards, encompassing advanced clinical assessment, critical thinking and clinical management skills that are evidence-based and deliver high-quality patient-centred care.

Main duties include:

• Planning and directing the management of patients including accessing services within the multi-disciplinary team and wider health, social care, education, vocational and voluntary sector services as appropriate.
• Leading on the strategic development of Trust-wide services in their specialist area, working with other strategic and clinical leads as appropriate.
• Providing peer support to clinical staff working with patients from the direct caseload or those involved in the management of patients.
• Assisting in the planning, co-ordinating and development of any newly implemented service plans.
• Ensuring that relevant legal and regulatory frameworks are acknowledged through local governance procedures.

Key Responsibilities include:

• Comprehensively assessing, reviewing and evaluating the needs of service users with undifferentiated healthcare needs to improve their physical, psychological and mental health wellbeing.
• Accurately prioritising patients within the service, undertaking clinical assessment and providing necessary treatment.
• Developing a plan of care in conjunction with the service user through application of clinical knowledge and analysis of symptoms and data.
• Utilising effective communication to aid onward referrals to specialists as appropriate.
• Maintaining patient confidentiality as per CityCare's clinical guidelines.

Essential qualifications include being a registered healthcare professional with a recognised full Masters level qualification in Advanced Clinical Practice and Independent Non-medical Prescriber (V300). Experience of working at an Advanced Practice level with patients presenting with undifferentiated illness is also required.
