As a Clinical Pharmacist at Marlow Medical Group, you will provide clinical pharmacy expertise across the practice, supporting safe, effective and sustainable primary care delivery. You will work closely with GPs, nurses, advanced practitioners, pharmacy colleagues, administrative teams and the wider multidisciplinary team to improve patient outcomes, support medicines safety, reduce avoidable hospital admissions, improve access to care and strengthen medicines optimisation across the organisation.
You will use your clinical expertise to support patients with long-term conditions, minor ailments, medication queries, structured medication reviews and post-discharge medicines reconciliation. You will also contribute to prescribing safety, repeat prescribing processes, quality improvement, clinical governance and the delivery of relevant primary care and PCN priorities.
The Clinical Pharmacist will provide direct care to patients with long-term conditions, minor ailments and other clinical needs within their professional scope of practice. You will undertake medication reviews and structured medication reviews, either face to face, remotely or, where appropriate, in a patient's place of residence. These reviews will be centred on each patient's individual needs and will support safe, effective, evidence-based and person-centred prescribing.

We are now looking for an enthusiastic and motivated Clinical Pharmacist to join our dynamic and evolving Clinical Pharmacy Team.
Job Responsibilities:
- Apply clinical expertise in Medication Reviews/Structured Medication Reviews (SMR) face-to-face, remotely or within a patient's place of residence, to deliver high-quality patient-facing services that are centred on and tailored to each individual patient's needs.
- Manage and deliver clinics in long-term conditions in collaboration with and support from allied healthcare professionals.
- Prescribe medication within the limits of the individual professional competence.
- Pro-actively and prospectively reduce inappropriate polypharmacy and over-diagnosis through systematic, evidence-based de-prescribing.
- Promote the principles of public health; educate and motivate patients to make healthy lifestyle choices and access services designed to assist health improvement.
- Discuss patients with complex needs at local MDT meetings and refer to the ICPT in line with recommendations.
- Provide professional telephone/email advice and support to patients and their carers.
- Reconcile the medications of patients whose care is transferred back to primary care in a timely and effective manner.
- Interface between community, hospital and primary care pharmacy colleagues as patients move between care settings developing local effective communication.
- Maintain full and complete records of all patient contacts using appropriate clinical templates and SNOMED coding.
- Support the effective utilisation of the practice workforce.
Person Specification:
Experience:
- Essential: Independent Prescriber, Primary Care Experience (either in a GP practice or a Primary Care Network), Long term condition management.
- Desirable: Managing Lab results, Menopause reviews, Respiratory reviews.
Qualifications:
- Essential: Pharmacy Degree, Registration with the General Pharmaceutical Council (GPhC), Centre for Pharmacy Postgraduate Education (CPPE) or Primary care Pharmacy Education Pathway (PCPEP).
- Desirable: Postgraduate Diploma in Clinical Pharmacy (or equivalent).
