Overview

In this role, you will lead the PCN Care Home Team to deliver high-quality, near-home care for residents in care homes. You will apply advanced clinical assessment and independent prescribing to manage acute, chronic, and complex needs, aiming to avoid unnecessary hospital admissions. You will provide clinical leadership, drive service transformation, and mentor staff to build capability across the team. You’ll work with GP practices, community teams, and care home providers to improve outcomes and support end-of-life care when appropriate. This is a strategic, hands-on clinical leadership role within the Primary Care Network.

Responsibilities

  • Provide day-to-day oversight of the Care Home Team and manage demand
  • Clinical leadership for the Care Home Service across the PCN
  • Identify, design, and implement clinical audits and QI projects
  • Support investigations of incidents, safeguarding concerns, and near-misses
  • Communicate complex medical information to patients, carers, and multidisciplinary teams
  • Participate in multidisciplinary reviews of high-risk residents
  • Develop personalised care plans with residents, families, and GP practices
  • Facilitate advance care planning and DNACPR discussions
  • Lead clinical governance and ensure compliance with standards
  • Lead and participate in clinical audits, QA programmes, and service evaluations
  • Deliver education and competency development for staff
  • Promote lifelong learning and evidence-based practice across care homes
  • Analyse service data to identify improvement opportunities
  • Support implementation of new care models for frail and elderly populations
  • Disseminate audit and research findings to teams and stakeholders

Key requirements

  • Registered healthcare professional with current registration (NMC/HCPC/GPhC)
  • MSc Advanced Clinical Practice (or equivalent)
  • Independent Prescriber qualification
  • Advanced clinical assessment, diagnostics and decision-making qualification
  • Experience across four pillars of ACP
  • Significant post-registration experience in primary/urgent/community/acute care
  • Experience leading MDT work, admission avoidance, and service development
  • Experience in supervision and education of staff
  • Excellent communication with patients, families, and colleagues
  • Strong organizational and prioritization skills
  • Ability to adapt workload to fluctuating demands
  • EMIS Web proficiency
  • Independent prescribing
  • Advanced clinical assessment and decision-making
