Overview
In this PCN Pharmacist role, you will integrate with a network of practices to deliver medicine management and primary care pharmacy services. You’ll work within the primary care team on agreed projects and move towards expanding your clinical role. The position supports independent prescribing where qualified and focuses on cost-effective prescribing, medication reviews, and audits. You’ll contribute to pharmacy-led initiatives across PCN, Federation, and practice levels, driving safer, evidence-based care for registered patients.

Responsibilities
- Implement an effective medicine management system within practice
- Provide medicine reviews for registered patients via multiple channels
- Reconcile medicines after hospital or clinic discharge
- Review safety alerts and ensure actions are taken
- Carry out targeted medication reviews for high-risk patients
- Review and implement safe prescribing systems for vulnerable patients
- Provide subject matter expertise on medication monitoring and systems
- Undertake PCN, Federation, and practice-level audits
Key requirements
- GPhC Registered Pharmacist
- GPhC Independent Prescriber Qualification (Desirable)
- Medicines Management Qualification (Desirable)
- MUR and repeat dispensing certification (Desirable)
- UK Registration (required)
- Minimum two years' experience in pharmacy (Desirable)
- Experience in managing pharmacy services in primary care (Desirable)
- Broad knowledge of General Practice (Desirable)
