Overview

In this role you will drive transformation across Brent ICP, mapping services and developing plans to co-locate and integrate care closer to residents. You will work with CNWL and partners to deliver integrated neighbourhood teams, ensuring outcomes align with community needs and cost-effectiveness. You’ll engage diverse stakeholders, champion co-production with residents, and help implement integrated pathways within a wider Northwest London ICS context.

Responsibilities

  • Map services to Brent's geographical footprints and develop delivery plans for five neighbourhoods
  • Identify opportunities to improve patient outcomes through service co-location and integration
  • Develop integrated pathways and align stakeholders on a shared vision
  • Lead stakeholder engagement to secure buy-in and manage information for PCNs and INTs
  • Foster co-production with residents and present ICT developments to GP forums/patient groups

Key requirements

  • Master's degree or equivalent
  • Project management qualification or proven delivery experience
  • Experience leading transformation projects in health and/or social care
  • Proven stakeholder engagement and co-production experience
  • Knowledge of financial flows or contractual setups related to PCNs/INTs (desirable)
