Overview
In this role you will join a multidisciplinary primary care team as a patient-facing Clinical Pharmacist across 2–3 practices.
You will lead medicines optimisation, manage chronic conditions, and contribute to DES and broader PCN priorities.
The role emphasizes collaboration with GPs, nurses, and hospital pharmacists to improve patient outcomes and reduce inequalities.

Responsibilities
- Clinically assess and treat patients using medicines knowledge in specific disease areas
- Prescribe or train to prescriber status and work with the general practice team
- Care management for chronic diseases and conduct clinical medication reviews
- Lead medicines optimisation and contribute to quality improvement and safeguarding antimicrobial stewardship
- Support integration of general practice with wider health care teams and improve patient access
- Develop relationships with pharmacy professionals across PCNs and health/social care system
- Lead shared care protocols, engage in clinical research, and liaise with specialist pharmacists
- Conduct structured medication reviews for polypharmacy, housebound patients, and priority conditions
- Reconcile medicines post-discharge and support post-discharge continuity
- Provide medicines information and education to practice staff and patients
- Perform medicine reconciliation, audits, and safety monitoring; respond to MHRA alerts and safety guidelines
- Support public health campaigns and signpost patients to appropriate services
- Key requirements
- Registered pharmacist with GPh C
- Minimum 2 years postqualification experience
- Independent prescribing qualification, or in training/intent to complete
- Understanding of clinical pharmacist role in primary care and ability to work in a PCN
