Intensive Outreach Specialist in London

We are seeking four enthusiastic and motivated Band 6 Community Mental Health Practitioners to join our newly established Intensive Outreach Service (IOT) supporting adults with complex mental health. This is an exciting opportunity to help shape a new service delivering intensive, recovery-focused interventions to individuals who require enhanced support in the community.

The post holder will work as a Care Coordinator within a multidisciplinary team, managing a defined caseload and providing comprehensive assessment, care coordination, risk management and evidence-based interventions. Working closely with Psychiatrists, Psychologists, Nurses, Social Workers, Occupational Therapists, Primary Care and voluntary sector partners.

You will undertake holistic biopsychosocial assessments, develop and review care and crisis plans, coordinate support across multiple agencies and provide assertive outreach to individuals who may be difficult to engage. The role involves delivering interventions in a range of community settings, including service users' homes, supported accommodation and community venues. You will monitor mental and physical health, manage risk, support discharge planning and help prevent unnecessary hospital admissions.

Using a recovery-oriented and trauma-informed approach, you will ensure interventions address the psychological, physical, social, cultural and spiritual needs of service users.

Main duties of the job:

• Act as Care Coordinator for a defined caseload of adults with complex mental health needs, providing assessment, care planning, care coordination and evaluation of person-centred interventions.
• Undertake comprehensive biopsychosocial, risk, mental capacity and safeguarding assessments, developing formulations that inform recovery-focused care.
• Develop, implement and review care plans, crisis plans, Dialog+ interventions and risk management plans in collaboration with service users, carers and partner agencies.
• Provide intensive outreach support in a range of community settings to promote engagement, recovery, independence and relapse prevention, reducing the need for hospital admission where appropriate.
• Deliver evidence-based interventions that address the physical, psychological, social, cultural and spiritual needs of service users.
• Work collaboratively with psychiatrists, psychologists, nurses, occupational therapists, social workers, primary care services and voluntary sector partners to ensure coordinated and effective care.
• Monitor mental and physical health needs, support medication management, evaluate outcomes and facilitate safe discharge planning and community reintegration.
• Respond effectively to mental health crises, safeguarding concerns and clinical risk.

Person Specification:

• Registered Mental Health Practitioner, Professional registration as RNMH/AHP/Occupation Therapist.
• Must be currently registered with appropriate professional body.
• Evidence of ongoing CPD in portfolio.
• Post registration experience of working in a mental health setting.
• Clinical Assessment Skills/Experience.
• Communication & MDT Working.
• Risk Management.
• Care Coordination.

Desirable:

• Registered Mental Health Nurse (RMN), Occupational Therapist, Social Worker or other relevant Allied Health Professional with current professional registration.
• Must maintain current registration with NMC, HCPC or Social Work England and comply with professional standards.
• Evidence of ongoing CPD and commitment to maintaining and developing professional knowledge and skills.
• Minimum of two years post qualification experience working within community or secondary mental health services with adults experiencing severe and enduring mental illness.
• Experience undertaking comprehensive mental health, risk and biopsychosocial assessments and developing recovery-focused care plans.
• Experience managing a complex caseload and coordinating care across multidisciplinary and multi-agency settings.
• Ability to communicate effectively with service users, carers and professionals and work collaboratively within a multidisciplinary team.
• Experience assessing and managing clinical risk, safeguarding concerns and mental health crises.
