Overview
As a Senior Discharge Lead, you drive complex discharge management within the Integrated Discharge Team to improve hospital flow. You provide clinical leadership, coordinate across wards, and oversee a team to resolve complex discharge issues. You will influence trust-wide discharge initiatives and quality improvements, collaborating with internal and external partners. The role offers impactful work improving patient experience during discharge and flow across sites.

Responsibilities
- Lead the complex discharge team on a day-to-day basis as part of the Integrated Discharge Leadership team
- Take overall responsibility for leadership, delivery and management of the complex discharge service
- Plan and organise patient management and time use efficiently
- Provide staff with practice support, development, supervision, appraisal and training
- Review and update discharge planning documentation and conduct annual audits
- Educate clinical and nursing staff on the remit of the Discharge team
- Work out of hours according to local rota
- Contribute to trust-wide discharge improvements and quality improvement projects
Key requirements
- Leadership and management experience in healthcare
- In-depth knowledge of National Discharge Guidelines
- Experience in complex discharge planning with clinical, operational and external partners
- Senior nurse or equivalent operational experience in acute, intermediate or community settings
- NMC registration and mentorship qualification
- Evidence of ongoing professional development
- Desirable postgraduate qualification or leadership development
- Excellent verbal and written communication
- Ability to form cross-sector relationships
- Adaptability and resilience
- IT systems competency, electronic patient records
- Interpreting data
- Knowledge of HR/Clinical Governance policies (e.g., attendance, capability, risks, complaints)
