Job summaryWe are looking for 1 WTE PCN Care Coordinator/HCA with excellent communication and administrative skills to work with the GPs and other clinicians within the Mawbey Group Practice - North Lambeth PCN.You will be working with GPs and other clinicians, to assist patients in the management of their care.This role will be based on the needs of the practice and support the work of the PCN at practice level.
