Overview
In this role you will provide comprehensive assessment, direct care, and targeted interventions for individuals with severe personality disorder within a community setting. You will work as part of the Personality Disorder Service Community Team under the Community Care Group, delivering evidence-based care aligned with NICE guidelines. You’ll handle a defined caseload, develop specialist treatment plans, and contribute to service evaluation and governance. You will collaborate with families and multisector teams to maximise patient strengths and safety, shaping care within a supportive NHS framework.

Responsibilities
- Attend and engage in individual and group supervision
- Assess, plan, implement and evaluate care with service users and carers
- Participate in clinical audit and service evaluation to improve care
- Contribute to MDT meetings with a specialist nursing/OT/HPC perspective
- Maintain up-to-date knowledge of legislation and local policies
- Maintain contemporaneous records to required standards
- Assess and manage risk continuously per trust policy
- Encourage service users’ responsibility for their care and involve carers where appropriate
- Support colleagues in assessment/management through data review and care reviews
- Identify carers and provide carers assessments and review plans
Key requirements
- Registered health care professional (e.g. NMC, HCPC, SWE)
- Knowledge/experience related to personality disorders/mental illness and evidence-based practice
- Self-awareness and reflective practice
- Ability to work effectively in crisis situations
- Collaborative teamwork
- Dialectical Behaviour Therapy (DBT)
