Overview
In this Clinical Pharmacist role, you will contribute to medicines optimisation across the Health Alliance PCN, supporting safe, effective prescribing for a patient population of over 58,000. You will work within a multidisciplinary team to handle medication reviews, chronic disease management, and prescribing safety, with scope for progression towards Independent Prescriber status. The role offers a collaborative, forward‑thinking primary care environment and the chance to make a tangible impact on patient care. You will engage in CPD and work toward autonomy in prescribing where agreed.
Responsibilities
- Provide structured medication reviews for patients with complex needs and polypharmacy
- Support medicines optimisation across the PCN and assist patients in safe, effective medicine use
- Give medication advice to patients, carers, clinicians, and practice teams
- Support chronic disease management through reviews, monitoring, and prescribing support
- Contribute to safer prescribing through audits, high‑risk drug monitoring, and safety alerts
- Support achievement of QOF, IIF, CRM, PCN DES and other contractual requirements
- Collaborate with GPs, nurses, technicians, care coordinators, social prescribers, and other MDT members
- Review hospital discharge and clinic letters; ensure safe medicines reconciliation
- Reduce medicines waste and inappropriate prescribing; ease patient medication burden
- Maintain accurate clinical records with proper coding/documentation
- Work within professional, legal, and ethical boundaries; escalate concerns
- Engage in CPD and pursue Independent Prescriber status if part of development plan
Key requirements
- Pharmacy registration (UK)
- Current UK professional registration
- Ability to drive and live within a 30‑minute drive from Harrow
