Overview
In this role you will work as an RMN within the Assertive Outreach pathway to engage adults with severe mental illness in the community. You will deliver time-limited, intensive support, focusing on stabilising care, reducing risk, and preventing crisis. You will act as a consistent keyworker within a multidisciplinary team, providing holistic, person-centred care and planning. This is an opportunity to shape community mental health practice through proactive outreach and collaborative care transitions.

Responsibilities
- Deliver assertive, flexible outreach including home visits and community contacts
- Act as dedicated keyworker with a small caseload (10β15 patients) over a 6-month model
- Conduct initial and ongoing risk assessments and manage escalation as needed
- Provide comprehensive mental health nursing care: assessment, formulation, care planning, and treatment
- Deliver evidence-based, CBT-informed interventions
- Address mental and physical health, housing, substance use, and social needs
- Coordinate interagency care plans and safe transitions
- Lead on step-down and discharge planning
- Maintain accurate clinical records, care plans, and reports
- Supervise Band 4 staff
- Work autonomously and within an MDT to ensure continuity of care
- Follow MHA, MCA, safeguarding frameworks
- Support reintegration into routine CMHH care
- Documentation & leadership responsibilities including care plan development and reporting
Key requirements
- Registered Mental Health Nurse (RMN) with post-registration experience
- Strong skills in assessment, care planning, and therapeutic engagement
- Expertise in risk assessment and management
- MDT and interagency working experience
- Knowledge of MHA, MCA, and safeguarding
